Important Reminders for 2026!

Biometric screenings are not required to begin earning credit, but members may choose to have a screening through their doctor. Members will earn credit for activities completed.

• Onsite screenings will not be available in 2026
• Earn Silver Status each by 8/31/26, to avoid a 15% disincentive for each employee and each covered spouse
• Spousal authorization is still required through the login process

Earning Opportunities

• Preventive Activities offered to ALL MEMBERS

  • Dental cleaning, once every year
  • Vision screening, once every year
  • Annual physical, once every year
  • Flu vaccine, once a program year
  • Tetanus vaccine, once every 10 years

• Preventive Activities offered to members based on their age and gender

  • Shingles vaccine shot series, once per lifetime for males and females aged 50 or older
  • Pneumonia vaccine shot series, once per lifetime for males and females aged 65 or older
  • Colon cancer screening, once a year for males and females 45 to 75 years old
  • Breast cancer screening, once every 2 years for females 40 to 74 years old
  • Cervical cancer screening, once a year for females 21 to 65 years old
  • HPV vaccine, once per lifetime, males and females 18 to 26 years old
• Preventive Activities offered to members based on responses to the onboarding and health assessments regarding medical or chronic conditions
  • Diabetes profile, once per program year, with a diabetes diagnosis
  • Diabetes appointment prep, once per program year, with a diabetes diagnosis
  • Diabetes doctor's visit, once per program year, with a diabetes diagnosis
  • Diabetes eye exam, once per program year, with a diabetes diagnosis
  • Diabetes foot exam, once per program year, with diabetes diagnosis
  • Recheck blood pressure, once per program year
  • GAD-7 Anxiety screening - coming soon!
  • PHQ-9 Depression screening - coming soon!
  • Hypertension appointment prep, once per program year, with a hypertension diagnosis
  • Hypertension doctor's visit, once per program year, with a hypertension diagnosis
  • 6-month A1c test, every 6 months, with a diabetes diagnosis
  • Quarterly A1c test, quarterly, with a diabetes diagnosis
  • Lung cancer screening, once per program year, for adults aged 50 to 80 who have a 20-pack-per-year smoking history and are currently smoking or have quit smoking within the past 15 years

Look back period for preventive activities is 9/1/2025 - 8/31/2026

A ‘covered employee’ refers to employees who are enrolled in Priority Health medical insurance through Ottawa County on January 1, 2026. Employees and spouses newly enrolled on the insurance plan on or after January 2, 2026, do not need to participate in the 2026 Health Management Plan. The spouse covered under the employee’s medical insurance is referenced as the ‘covered spouse’.
