Additional Dependent Life Insurance

Additional dependent life insurance may be elected during initial insurance enrollment or annually, during open enrollment.

For purposes of dependent life insurance, Child is defined as: Your natural child, adopted child (including a child from the date of placement with the adopting parents until the legal adoption) or stepchild and who, in each case, is at least 15 days old, under age 26, unmarried and supported by you. The term does not include any person who:

  • is on active duty in the military of any country or international authority; however, active duty for this purpose does not include weekend or summer training for the reserve forces of the United States, including the National Guard; or
  • is insured under the Group Policy as an employee

Available options:

  • Option 1: $10,000 spouse/$5,000 for $0.87 per pay period
  • Option 2: $15,000 spouse/$7,500 children for $1.31 per pay period
  • Option 3: $20,000 spouse/$10,000 children for $1.73 per pay period

MetLife Supplemental Benefit

MetLife also provides employees with the option to purchase additional employee and dependent term life insurance, at cost to the employee.  This is in addition to dependent life insurance elected during initial insurance enrollment or during open enrollment. Term Life insurance can provide you and your family with financial security in the event you or a covered person passes away. Term Life Insurance is provided at an inexpensive rate while offering a range of benefit amounts to fit you and your families needs. This term life insurance is in addition to your current life insurance and your elected dependent life insurance through MetLife. Additional life insurance is available for:

  • Employees, up to $100,000
  • Spouses, up to $25,000
  • Children, up to $10,000

Please note: This policy is offered to all newly hired eligible employees on an initial GUARANTEE ISSUE basis and no health questions are required. If no election for coverage is made during your initial eligibility, future enrollment will require a medical questionnaire and you may be denied coverage. Changes during the plan year are subject to a qualifying life event. This supplemental benefit is not administered by Ottawa County. Any agreements made with MetLife (regarding additional term life insurance) as a result of this information is solely between the employee and MetLife, and is at cost to the employee.

When electing this supplemental Term Life Insurance, these additional benefits are available to you:

  • Estate Resolution Services
  • Estate Planning Handout
  • Travel Assistance
  • Funeral Planning Assistance

To access the will preparation service, call Hyatt Legal at 800-821-6400, and let the representative know that you are not a Hyatt Legal member, but will preparation is a benefit with your supplemental life insurance through MetLife. Hyatt will take your information and give you a case number and a list of local attorneys. Provide your case number to the attorney you choose.

For general questions, please contact Erin Rotman at 616-738-4805 or erotman@miottawa.org. All enrollment and cancellation forms for MetLife should be sent to Human Resources.
